Where do I send a appeal?

I was banned because of my name *BananaSuckler22* the only reason I have the name is because my friend changed it as a prank, but its been getting me in a lot of trouble on servers and this was the last one I could play. Ill just liked to be unbanned please and Ill change it back as soon as I can.
